The Clinical Nurse Consultant (CNC) coordinates care across the full cardiology patient journey, including inpatient, outpatient, emergency, elective, and transfer cases. Acting as a key point of contact, the CNC supports communication between patients, families, and healthcare teams, while guiding patients through treatment and providing education. As part of the multidisciplinary team, the CNC uses advanced clinical expertise to lead care delivery, make independent decisions, and improve service outcomes. The role focuses on enhancing continuity of care, patient experience, and overall clinical outcomes.
