The Transition and Post Care Support Officer will take a lead role in directly supporting young people with complex and high risk behavioural, emotional, psychological, disability and mental health needs who are most at risk of homelessness while they transition to adulthood. Your work will focus on young people’s access to appropriate housing, behaviour support and family and support networks, the development of independent living skills, integration into their local community and links to specialist services, mental health and general health services, counselling services and employment activities.
