This role will support the implementation of a novel urgent community aged care paramedicine model of care within Population & Primary Health Care, designed to strengthen the interface between community, primary care and hospital services by providing timely, proactive care to older people, reducing avoidable hospital use and supporting care closer to home. Selection will be made at the N5 or N6 level depending on the qualifications of the successful applicant.
