The Continuing Care Teams (CCT) provide community based mental health assessment, treatment and support for adults (18-65 years) and their families and/or carers living with mental illness. Join a multi-disciplinary teams to provide specialised mental health assessments and interventions in a culturally diverse population to enhance community integration and networking with available support agencies. • Permanent part time opportunity – 38 hours per fortnight!
