This role provides advanced nursing care using a case management model and coordinates comprehensive patient care. The Nurse Practitioner delivers advanced assessment and management, provides emergent responses to Residential Aged Care Facilities to prevent ED presentations, and supports community‑based care, including care in patients’ homes, as part of an interdisciplinary team.
