Join a dedicated team providing enhanced culturally safe chronic conditions health services! The Mūrrūmba Targan Djimbulung Service provide wrap-around health and well-being support to First Nations adults who live in West Moreton and experience chronic health issues, such as diabetes and going heart and lung issues. The purpose of the role is to provide high level clinical leadership working within a multidisciplinary environment, delivering evidence-based, clinical social work services for Aboriginal and Torres Strait Islander men, women, families and carers participating in the Mūrrūmba Targan Djimbulung Service. Full Time Temporary Position for up to 12 months!
