Advance Aged Care as a Nurse Practitioner Join Metro South Health's CAREPACT team as a Nurse Practitioner and deliver advanced, patient-centred care for older people living in residential aged care. Based at Princess Alexandra Hospital, you'll work collaboratively across healthcare settings to improve outcomes, reduce avoidable hospital presentations, and ensure patients receive the right care, at the right time, in the right place. This is a temporary part-time opportunity (up to 40 hours per fortnight for 6 months).
