As a senior pharmacist, you will deliver high-level clinical pharmacy services to ensure medication safety across the continuum of care within the Community and Oral Health (COH) directorate. COH leads in transitions of care, offering services such as inpatient rehabilitation, Hospital in the Home, and face-to-face pharmaceutical care in patients’ homes.
