The purpose of this role is to deliver Social Work clinical services as a part of a rotational arrangement to provide optimal, responsive, and flexible Social Work services in acute and community settings. You will work in a multidisciplinary environment and have a core focus on delivering clinical services across the continuum of care. This role will include a focus on delivering interventions for patients with range of diseases, and a caseload focussing on strategies to maintain and improve health, functional ability and quality of life, and admission avoidance.
