The Senior Social Worker will deliver clinical social work services as a part of a multidisciplinary Memory and Cognition clinic and general geriatrician outpatient clinics. The position will provide leadership in the coordination and delivery of Social Work services to adults with a suspected diagnosis of young onset dementia or cognitive decline through contributing to timely assessments, post diagnostic interventions and education with adults and their partner, carer, or support person. •Permanent Part-time opportunity – 38 hours per fortnight!
