The Transition and Post Care Support (TPCS) Officer provides direct support to young people with a care experience aged 15-21 years with complex support needs (e.g. disability, significant mental health concerns and/or high-risk behaviours) who are most at risk of homelessness as they transition to adulthood and post-care. Your work will focus on the development of protective independent living skills, family and support networks and access to necessary specialist supports relevant to the young person’s risks and needs. The TPCS Officer works in collaboration with Child Safety staff, government and non-government agencies to improve young people’s quality of life, achieve stability in their living situation, and increase their involvement in schooling, employment, their family, culture and community.Refer to the role description for more information.
